CAVEAT EMPTOR
As most patients know implants come in round and tear drop shapes. They are filled with saline or silicone. Their outer shells are either smooth or textured. In recent years new concerns have arisen about textured tear dropped silicone filled implants. At a recent ASAPS national meeting in Las Vegas a panel of famous plastic surgeons had some interesting things to say about these implants.
First of all, even plastic surgeons cannot tell if an implant is round or tear drop shaped when it is inserted into a patient’s body. This means tear dropped shaped implants have no 3 dimensional advantage over round implants.
Second, a significant percentage of these textured implants do not “stick” to the surrounding breast tissue. If they do not “stick” then they can move. This then creates an odd looking breast that can look like it is sitting sideways or upside down. The shape now becomes unacceptable.
Third, any implant placed under the breast and on top of the pectoralis muscle that does not “stick” has a much higher risk of capsular contracture where the breast can get rock hard and look real bad.
Finally there is a small risk that any patient with a textured shaped or round silicone implant is at risk for the development of cancer. The cancer is ALCL—Anaplastic Large Cell Lymphoma. Most patients can be cured of this disease but not all. Some people die of ALCL.
The conclusion of the panelists was that they had stopped using textured round or tear drop shaped implants. They both had switched back to round smooth silicone implants placed under the muscle. There is no cancer risk with these smooth implants or other risks such as an increase in capsular contracture (hard and distorted looking/feeling implant). There is no risk of developing a breast deformity because no matter how these implants move they always look the same (round).
Many American women have had round or shaped textured silicone implants and have gotten good results and are happy. The risk of cancer is very low but real.
In my opinion there are other problems with round/shaped silicone implants. They are significantly more expensive than round implants with no real advantages and multiple disadvantages.
Mammograms are easier to read when the implants are placed under the muscle. Most textured round/shaped silicone implants are put on top of the muscle. I believe this puts these women at increased risk of delayed diagnosis of breast cancer meaning their risk of dying from breast cancer goes up. The risk of capsular contracture is lower with implants under the muscle.
I believe the risk of capsular contracture or unnatural firmness is higher with the round/shaped textured silicone implants placed on top of the muscle as compared to round smooth implants placed under the pectoralis muscle.
The bottom line is that each patient must do their homework before deciding what type of implant they want to have placed into their body, where they want the incision placed, how big they want to be, and whether or not they feel comfortable with round/shaped textured silicone implants. It is very possible that a patient could be very pleased with a round/shaped textured implant placed on top of the muscle but the foregoing discussion lays out the thinking of some of the top breast experts in the world so be focused when you are making your choices.
Easy Diet Tips:
Here are some east diet tips that if you follow you will be guaranteed to lose weight in a healthy manner and will be more likely to keep the weight off once you are where you want to be.
1. Drink at least 80 ounces of water per day. Drink 8oz. ice cold water prior to each meal. Studies show this will result in weight loss compared to patients who eat the exact same diet.
2. Chew each bite of food 7-10 times before you swallow. Start with the healthy foods first and save the starchy/ fatty foods for later. You will fill up well before you have cleared your plate. This will result in tummy shrinkage and the desire to eat smaller servings.
3. Try to eat dinner no later than 6pm. Do not snack at all after your meal. This keeps your blood insulin levels low and your blood growth hormones high. This hormone balance is best for anti-aging, weight loss, and prevention of chronic disease.
4. Begin to learn to eat better. Cut out all unnecessary starches. Shop the green grocer part of the grocery store and learn to eat fruits and vegetable you have never tried. Use the internet to look up each food and educate yourself about what you are eating. This is very easy to do.
5. Try to get some exercise 3-5 times per week. Simple walking is a good start. Bike riding or swimming is good. You can join a gym for a more intense workout and pay for a trainer to help you put together a routine that works for you.
